- In reference to the to the DepED Information and Communications Technology Service Memorandum Dated March 17, 2017, Submission of School User Account Creation Forms (UACF) for the Creation of School Personnel User Accounts in EHRIS v2.
- All forms and instructions can be downloaded thru: bit.ly/ehris_School-Template
- All Elementary, Secondary and SHS School ICT Coordinators in coordination with the School Heads shall download the corresponding School UACF Form and fill up necessary required information.
- All accomplished forms shall be submitted to the Division IT Officer thru email: [email protected] or Division Office Email: [email protected]
- SDO HRMO in coordination with the SDO ITO will help disseminate and submit the consolidated UACF Forms to Central Office on ICTor before March 31, 2017.
