Submission of pertinent documents to the Schools Division Office shall be until April 17, 2017. DepEd Order no. 7, s. 2015 shall be used as the basis of the criteria during the ranking.<\/p>\n<p>3. Sharon Teh- Ylanan<\/p>\n<p>4.Applicants are advised to include the following documents in their pertinent documents:<\/p>\n<ul>\n<li>CSC Form 212<\/li>\n<li>Certified photocopy of PRC professional ID, or PRC Certification showing teacher\u2019s name, LET Rating, and other information<\/li>\n<li>Certified Photocopy of Ratings \u2013 LET or PBET<\/li>\n<li>Certified Photocopy of the Transcript of Records<\/li>\n<li>Copies of the Service Records, Performance Rating and School Clearance for those with Teaching Experience<\/li>\n<li>Certificate of Specialized Training<\/li>\n<li>Certified Copy of Voter\u2019s ID or\/ any proof of residency deemed acceptable by the school screening committee<\/li>\n<li>NBI Clearance<\/li>\n<li>Omnibus Certification of authenticity and veracity of all the documents submitted.<\/li>\n<\/ul>\n<p>5.
