The survey aims to gather \/ collect data which are vital in formulating and enhancing health related policies and programs of PhilHealth specifically to Central Luzon populace.<\/li>\n<\/ol>\n<ol start=\"2\">\n<li>Relevant to this matter, this office is requesting you to extend the utmost assistance to PhilHealth in conducting the said activity through distribution of the survey forms\/ tools on the 4<sup>th<\/sup> week of February and collecting the same from the pupils on the 3<sup>rd<\/sup> week of March 2017. The collected forms shall be submitted to this office on the 4<sup>th<\/sup> week of March 2017, attention: SGOD Office.<\/li>\n<\/ol>\n<ol start=\"3\">\n<li>Attached herein are the survey forms for reproduction.<\/li>\n<\/ol>\n<ol start=\"4\">\n<li>Immediate dissemination on the content of this memorandum is desired.<\/li>\n<\/ol>\n<p>&nbsp;<\/p>\n<p><strong>Engr.
